FINAO is pleased to announce the availability of its SOAP application that allows for doctors to simply point and click generating a progress note and patient encounter on the fly.
The term SOAP refers to a particular format of recording information regarding treatment procedures. Documentation of treatment is an extremely important part of the treatment process. In virtually all medical practice settings, some form of documentation is required and SOAP notes are the most popular format in medical settings.
With SOAP notes in mind, doctors need to be able to electronically capture patient visits. With this proven easy to use application, there is no need to dictate, greatly reduce transcription, and start the billing process immediately.

• Provide a more thorough examination and documentation.
The doctor simply walks through an encounter with the patient:
Subjective - This part of the notation should describe the clinician’s impression of the client/patient. This section should be utilized to report subjective information of clinical significance gathered from the patient. Here general information, chief complaint, previous treatments, review of systems, etc. is captured.
Objective - This section is where the doctor will report the measurable and observable information obtained during the treatment session. Common tests and site specific tests are used to gather the objective information.
Assessment - This section is where the doctor assesses, in descriptive terms, the patient’s performance during the session and/or the session itself. This section is where the doctor selects the diagnosis. Also in this section the doctor can order such things as x-rays, diagnostic tests (mri, ct, etc.) and labs.
Plan - The final section of your SOAP note is where the doctor outlines the course of treatment, after considering the information gathered during the session. Here the doctor can prescribe medicine, physical therapy, order surgery, injections, supplies, etc.
Immediately the practice has a summary or progress note of the encounter, CPT codes, procedure codes, any orders, any prescriptions, and the level of service.
One of the Orthopedic doctors currently using the system stated, “What else could a doctor want?”
The SOAP application also enables the staff and administration to get information previously difficult to come by. For example, medication logs are generated on the fly as prescriptions are created. All surgeries and diagnostic tests ordered, all injections and castings, are all captured in a log easy to retrieve and understand … available anywhere in the practice.
